TCA CROSS
A tiny scar can run much deeper than it looks.
TCA CROSS is a highly targeted technique designed primarily for ice-pick acne scars and selected deep, narrow atrophic scars.
Instead of resurfacing the entire face, treatment is concentrated inside individual scars to stimulate a controlled remodeling response where it is actually needed.

What you see on the surface is only part of the scar.
Acne scars are not all the same.
An ice-pick scar may have a tiny opening at the surface while extending substantially deeper into the dermis. That architecture is very different from a broad rolling scar or a boxcar depression.
This is why treating every acne scar with the same device or procedure can produce disappointing results.

TCA CROSS may be considered for:

Different scars need different treatments.
Most patients with meaningful acne scarring have more than one scar morphology. Treatment works best when each structural problem is addressed appropriately.
Ice-Pick Scars
Narrow, deep scars that can extend substantially into the dermis. These are the classic scars for which focal TCA CROSS is often considered.
Rolling Scars
Broad depressions that may be held down by fibrous attachments beneath the skin. These frequently require a different strategy such as subcision .
Boxcar Scars
Wider depressions with more defined edges. Treatment depends on their depth and architecture and may incorporate resurfacing or other scar-directed techniques.
Treat the individual scar—not the entire face.
CROSS stands for Chemical Reconstruction of Skin Scars. Trichloroacetic acid is placed focally within selected scars to create a controlled injury and subsequent remodeling response.
Identify
Appropriate scars are identified based on morphology, depth and the overall scar pattern.
Target
TCA is carefully placed into the selected scar rather than broadly applied over the face.
Remodel
The controlled injury initiates a healing and remodeling response within the treated scar.
Reassess
Improvement develops gradually. Additional sessions or complementary treatments may be considered based on response.
The goal is remodeling—not simply peeling the surface.
The depth and narrow architecture of an ice-pick scar helps explain why superficial treatments alone may not adequately address it.
TCA CROSS concentrates treatment inside the defect and allows the surrounding skin to remain largely untreated.
Remodeling occurs gradually over time. Treatment number, interval and whether another procedure should be incorporated depend on the individual scar pattern and clinical evaluation.
Most acne scar patients don't need one treatment.
They need the right treatments in the right sequence.
A patient may have ice-pick scars, tethered rolling scars, boxcar scars, pigment changes and generalized textural damage at the same time.
That is why acne-scar treatment frequently “ping-pongs” between structural correction, focal scar treatment and broader collagen remodeling.

Is TCA CROSS a chemical peel?
Not in the conventional full-face sense. Although trichloroacetic acid is used, CROSS is a focal scar procedure in which treatment is concentrated within selected individual scars.
Is TCA CROSS mainly for ice-pick scars?
Ice-pick scars are a classic application for TCA CROSS because they are narrow and can extend deeply into the dermis. Selected other narrow atrophic scars may also be considered after clinical evaluation.
Does TCA CROSS treat rolling scars?
Rolling scars may involve fibrous tethering beneath the skin. When tethering is contributing to the depression, a structural treatment such as subcision may be more relevant. Many patients have mixed scar patterns and may require multiple approaches.
Can TCA CROSS and subcision be combined?
They may both be incorporated into an acne-scar treatment strategy when different scar morphologies are present. They address different structural problems and do not perform the same function.
How many TCA CROSS treatments will I need?
There is no universal treatment number. Scar depth, morphology, response, skin type and whether other modalities are being used all influence treatment planning.
Can darker skin types undergo TCA CROSS?
CROSS techniques have been studied in a range of Fitzpatrick skin types. Pigmentary risk, including post-inflammatory hyperpigmentation, must be considered individually along with preparation and aftercare.
