Treatment
Ice-pick, boxcar and rolling acne scars may look similar from across the room, but they are not the same problem underneath the skin.
The goal is not to force every scar through one machine or one procedure. We identify the scar structure first, then select the treatment or combination of treatments that makes sense for that pattern.
Different scars require different strategies.
Many patients have more than one acne-scar pattern. Understanding the depth, shape and underlying structure helps determine which treatment modalities may be considered.
Ice-Pick Scars
Deep, narrow defects with a small surface opening that may extend substantially into the dermis.
Often considered: TCA CROSS and other focal scar techniques.
Boxcar Scars
Round or oval depressions with more defined edges. Boxcar scars can be relatively shallow or extend more deeply.
Often considered: resurfacing, microneedling, RF microneedling and selected combination approaches.
Rolling + Tethered Scars
Broad or wave-like depressions may occur when fibrous attachments underneath the skin pull the dermis downward.
Often considered: Subcision followed by collagen-remodeling or resurfacing strategies when appropriate.
The surface is only part of the scar.
A depression visible on the surface may be influenced by structures deeper in the dermis. Rolling scars can be tethered by fibrous bands, while ice-pick scars can extend much deeper than their small opening suggests.
Scar morphology, depth, skin type, active acne, pigment history, prior treatments and healing characteristics all matter when developing a treatment strategy.
Acne marks are not always acne scars.
Post-inflammatory hyperpigmentation and post-inflammatory erythema may remain after acne without producing a structural depression. Pigment and redness require a different strategy than true atrophic acne scarring.

This is why acne-scar treatment may be staged. A procedure that releases tethering is solving a different problem than a focal treatment for a deep ice-pick scar or a resurfacing treatment for broader texture.

Rolling or tethered scars may first require treatment of the attachments pulling the skin downward.

Deep focal defects may require treatment directed specifically into the scar rather than across the entire face.
TCA CROSSRemodel + Resurface
Broader texture and collagen remodeling may then be addressed with RF microneedling, HALO, CO₂, microneedling or other selected modalities.
COLLAGEN + TEXTUREAcne-scar treatment is not one-size-fits-all.
Skin tone, pigment response and history of post-inflammatory hyperpigmentation can influence treatment selection, preparation, energy settings, recovery planning and sequencing.
This is especially important when combining focal procedures with resurfacing or energy-based treatments.
Why an evaluation matters
Photos and text messages can be helpful for general education, but they cannot replace an examination of scar depth, tethering, skin characteristics and pigment risk.
The appropriate way to determine an individualized treatment plan is through a clinical consultation and evaluation.

Published literature describes acne scarring as a heterogeneous condition. Treatment selection therefore depends on scar subtype, depth and individual characteristics, and combination approaches are commonly discussed.

Can acne scars be completely removed?
Acne-scar treatments are intended to improve scar depth, texture and visibility. Complete elimination cannot be promised. Results vary according to scar morphology, depth, skin characteristics, healing response and treatment plan.
How do I know whether I have ice-pick, boxcar or rolling scars?
These terms describe different acne-scar morphologies. Photos can provide general information, but determining scar type, depth, tethering and the appropriate treatment requires clinical evaluation.
What is Subcision best suited for?
Subcision is commonly considered for selected rolling or tethered atrophic acne scars in which fibrous attachments underneath the skin contribute to the depression.
What is TCA CROSS best suited for?
TCA CROSS is a focal technique commonly used for ice-pick scars and selected deep, narrow atrophic scars. It is different from applying a chemical peel across the entire face.
Why might acne scars require more than one treatment?
One person may have several scar morphologies simultaneously. Subcision, TCA CROSS, RF microneedling and laser resurfacing address different structural components, so a staged combination may be considered after evaluation.
Should active acne be treated before acne scars?
Ongoing inflammatory acne may create additional scars. Management of active acne is therefore commonly considered as part of the overall acne and scar strategy.
