Bolus Med Spa is a boutique, appointment-based medical aesthetics practice. Our policies protect reserved clinical time, promote transparent financial expectations and allow our team to provide safe, individualized care.
Appointments
Your appointment time is reserved specifically for you.
Appointment-Only Practice
Bolus Med Spa operates by appointment. Walk-ins are not accepted. Appointments should be scheduled and managed through our approved booking system whenever available.
Patients are responsible for maintaining accurate contact information and reviewing appointment confirmations, preparation instructions and communications related to their scheduled care.
Arriving Late
Please text or call us as soon as possible if you are running late. Arriving more than 10 minutes late may require your appointment to be shortened, rescheduled or canceled when there is insufficient time to provide treatment safely and appropriately.
Appointments canceled or rescheduled with less than 24 hours' notice are subject to a $75 fee.
Missed appointments without appropriate notice are considered no-shows and are subject to a $75 no-show fee.
Repeated Late Cancellations or No-Shows
Repeated late cancellations or missed appointments may result in prepayment requirements, scheduling restrictions, loss of promotional pricing or Bolus declining to schedule future elective services.
Promotional Appointments
Promotional appointments are subject to the same cancellation requirements. Missing or late-canceling a promotional appointment may result in loss of promotional pricing in addition to the applicable cancellation fee.
Payments
We want treatment costs and payment expectations to be clear before services are provided.
Payment at Time of Service
Payment is due at the time of service unless another arrangement has been approved in advance.
Credit & Debit Cards
A 3.5% convenience fee applies to payments made using a credit or debit card.
Insurance
Bolus Med Spa does not accept or bill health insurance.
HSA/FSA cards may be accepted for eligible transactions. Eligibility, reimbursement and tax treatment are determined by the patient's plan administrator and applicable requirements.
Third-Party Payments
A spouse, parent or other third party may pay for another patient's services when permitted by Bolus. Recurring use of a payment method belonging to another person may require separate authorization from the cardholder.
Paying for another person's care does not provide the cardholder access to that patient's medical information or authorize disclosure of protected clinical information.
Packages, Prepaid Treatments & Treatment Credits
Unless otherwise stated in writing, treatment purchases, packages and treatment credits are intended for the individual patient for whom they were purchased and are nontransferable.
Any applicable use period, expiration, package conditions or other material restrictions will be disclosed at purchase.
Clinical Appropriateness Comes First
Purchasing or prepaying for a treatment does not guarantee that the treatment will remain clinically appropriate.
Recommendations may change based on medical history, examination, contraindications, response to previous treatment, medications, changes in health or other clinical considerations.
Financial arrangements never override the independent clinical judgment of a Bolus healthcare professional.
Your money stays yours.

Private Client is designed for patients who want an ongoing relationship with Bolus, preferred pricing, treatment planning and access to Private Client privileges.
Foundation
Elevated
How Private Client Works
Successfully collected Foundation and Elevated monthly contributions are credited to the client's Bolus Wallet in accordance with applicable Wallet terms.
Private Client status also provides access to applicable Private Client pricing and privileges. Specific pricing, benefits and treatment eligibility remain subject to the applicable agreement and clinical appropriateness.
12-Month Commitment
Foundation and Elevated require an initial 12-month contractual commitment.
Automatic Renewal
Following the initial term, the agreement renews for another 12-month term unless nonrenewed or canceled in accordance with the agreement and applicable law.
Recurring Payment
A valid authorized payment method must remain on file during participation.
Written Notice
Cancellation and nonrenewal requests should be submitted in writing in accordance with the applicable Private Client Agreement and applicable law.
Early Termination
Early termination during an incomplete contractual term is subject to the applicable fixed early-termination charge.
Foundation and Elevated do not add treatment-by-treatment discount recapture to these fixed Private Client early-termination charges. Properly accrued account balances remain separately due.
Past-Due Accounts
A declined, locked, blocked, expired, replaced or otherwise unavailable payment method does not itself constitute cancellation of a Private Client Agreement.
Private Client privileges and preferred pricing may be suspended while an account is past due.
Only successfully collected Foundation or Elevated monthly contributions are added to the Bolus Wallet. An unpaid contribution does not create Wallet credit.
Administrative Termination for Nonpayment
An account that remains unresolved for approximately 90 days following the first missed required payment may be administratively terminated for nonpayment.
Administrative termination ends future participation and future recurring program charges as of the effective termination date. Administrative termination does not automatically erase amounts properly accrued before termination.
Returning After Financial Default
A former Private Client whose account was administratively terminated for nonpayment is not automatically eligible to rejoin Private Client.
Outstanding financial obligations from a previously terminated Private Client account must be resolved before Bolus will provide additional elective services or consider future Private Client eligibility.
Resolution of a prior account does not guarantee future acceptance into Private Client. If re-enrollment is approved, the patient must enter a new agreement under the pricing, terms and eligibility requirements then in effect.
These financial policies do not prevent clinically appropriate follow-up related to care already provided when such follow-up is professionally or legally required.

TOX Access
TOX Access is a separate preferred-pricing program for patients primarily interested in neurotoxin treatment. It is not a Foundation or Elevated Private Client level.
$150 Minimum Treatment Charge
A minimum neurotoxin treatment charge of $150 applies per treatment visit.
The $25 monthly TOX Access fee is an access fee. It does not:
- enter the Bolus Wallet;
- apply toward the $150 treatment minimum;
- accumulate as treatment credit; or
- have cash value.
12-Month Agreement
TOX Access requires a 12-month contractual commitment and is subject to the renewal, recurring-payment, cancellation and nonrenewal provisions contained in the TOX Access Agreement.
Early Termination
If TOX Access is terminated before completion of the current contractual term under circumstances triggering early termination, the client is responsible for repayment of the neurotoxin pricing discounts actually received during that incomplete term, as described in the TOX Access Agreement.
For example, if the standard neurotoxin price on the treatment date is $15 per unit and the TOX Access price paid is $10 per unit, the conditional pricing difference is $5 per unit. One hundred qualifying units would therefore represent $500 in conditional pricing benefit.
Discounts from previously completed contractual terms are not included in early-termination recapture.
Legacy Silver, Gold & Platinum Programs
Silver, Gold and Platinum memberships are closed to new enrollment.
Bolus is no longer accepting new patients into these legacy programs. Existing participants remain subject to the terms applicable to their individual agreements while those agreements remain in effect.
Current ongoing-access options are offered through TOX Access and Private Client | Foundation and Elevated.
Refunds & Returns
Completed Services
Completed treatments and professional services are nonrefundable. Individual response and results vary. Payment is for the professional service provided and does not constitute a guarantee of a particular aesthetic or clinical outcome.
Prepaid Services & Packages
Prepaid services and packages are nonrefundable and nontransferable unless otherwise stated in writing or required by applicable law.
Retail Products
Unopened, unused and sealed eligible retail products may generally be returned within 30 days of purchase, subject to product-specific and legal restrictions.
Prescription products, opened products and products that cannot legally or safely be returned are not eligible for return or exchange.
Consultations & Virtual Discovery
Patients who are unsure where to begin may be offered a brief complimentary Virtual Discovery when available.
Virtual Discovery is intended to help identify a primary concern, determine whether Bolus may be an appropriate fit and identify the appropriate next step.
It does not establish an individualized treatment plan and does not replace an appropriate clinical evaluation.
Complex concerns may require an in-person clinical consultation, medical assessment, skin analysis or other evaluation before treatment recommendations can be finalized.
Any applicable consultation fee will be disclosed before booking.
Children & Guests
Our treatment environment contains medical equipment, lasers, injectables and other clinical materials.
For safety and to allow our clinical team to focus fully on patient care, children should not accompany patients into treatment appointments.
If appropriate supervision cannot be arranged and treatment cannot safely proceed, the appointment may need to be rescheduled and applicable cancellation policies may apply.
Guests may be restricted from treatment areas when necessary for patient privacy, safety, space or clinical considerations.
Digital & AI-Assisted Communications
Bolus may use automated and AI-assisted systems to help with general questions, scheduling information, appointment reminders, follow-up communications and other administrative functions.
Automated communication systems provide general educational and administrative information only.
They Do Not Replace Clinical Evaluation
Automated systems do not diagnose medical conditions, prescribe medication, establish individualized treatment plans or replace evaluation and clinical judgment by an appropriate healthcare professional.
Use of a website form, automated chat, SMS interaction or other general communication channel does not by itself establish a healthcare provider-patient relationship.
Do not rely on website chat, SMS, email or automated systems for emergencies or urgent medical concerns. Seek appropriate emergency or urgent medical care when necessary.
Privacy & Medical Information
Bolus takes patient privacy seriously. Clinical information is handled through appropriate systems and in accordance with applicable privacy requirements.
General website forms, marketing forms, email and automated chat should not be used to transmit urgent or highly sensitive medical information unless specifically directed by Bolus through an appropriate secure method.
Payment by a spouse, parent, partner or other third party does not, by itself, authorize Bolus to disclose the patient's medical information to that person.

Information provided on the Bolus Med Spa website and through general educational communications is intended for informational and educational purposes.
Website information is not individualized medical advice and does not constitute diagnosis, prescribing or an individualized treatment recommendation.
Determining whether a treatment is appropriate for a particular patient requires appropriate clinical evaluation.
Results Vary
Treatment descriptions, photographs, testimonials and examples may illustrate individual experiences or potential treatment objectives. They do not guarantee that another patient will achieve the same or any particular result.
Treatment Eligibility
Final treatment eligibility, technique, product selection, dosing, treatment interval and other clinical decisions remain subject to professional assessment and independent clinical judgment.
Safe & Respectful Practice
Bolus is committed to maintaining a safe, respectful and professional environment for patients and staff.
Consistent with applicable law and professional obligations, Bolus may establish reasonable requirements for future elective services when there is a history of repeated no-shows, unresolved financial obligations, payment disputes, abusive or threatening conduct, safety concerns or material violations of practice policies.
Financial or administrative restrictions do not supersede appropriate professional obligations related to clinically necessary follow-up for care already provided.
Vendors & Sales Representatives
Bolus Med Spa does not accept unscheduled vendor visits, in-office solicitation or solicitation of patients or staff.
Representatives seeking to introduce a product or service should contact our team electronically and provide their company, product or service, purpose of contact and contact information.
Submission of an inquiry does not guarantee an appointment or meeting.
