Hyperhidrosis is excessive sweating beyond what the body needs for normal temperature regulation. In appropriately selected patients, botulinum toxin can temporarily reduce sweat production within a treated area by interrupting the nerve signaling that activates sweat glands.

Understanding Hyperhidrosis

There is normal sweating - and then there is excessive sweating.

The first step is determining whether the pattern looks like primary focal hyperhidrosis or whether sweating could be a symptom of something else.

Primary Focal Hyperhidrosis

Sweating is the condition.

Primary focal hyperhidrosis typically affects one or more specific areas, often the underarms, palms or soles, and can occur even when the person is not overheated.

Secondary Hyperhidrosis

Sweating may be a symptom.

New generalized sweating, night sweats or a major change in sweating pattern can sometimes be associated with medications, endocrine or metabolic conditions, menopause, infection or other medical causes. When the pattern appears secondary, identifying the cause matters more than simply treating the sweat glands.

Botulinum toxin has been studied for palmar hyperhidrosis and may substantially reduce sweating in selected patients. Treatment requires additional counseling because injections can be uncomfortable and temporary hand weakness can occur.

Botulinum toxin has also been used for plantar hyperhidrosis. Treatment of the soles can be uncomfortable because plantar skin is highly sensitive, and the evidence base is smaller than for the underarms.

How Treatment Works

The sweat gland does not disappear. Its signal is temporarily reduced.

Plantar treatment can be particularly sensitive. Comfort planning is important, and the evidence base is smaller than for axillary treatment.

When sweating deserves a different evaluation.

New generalized sweating, drenching night sweats or a major unexplained change in sweating should not simply be assumed to be primary focal hyperhidrosis. Appropriate medical evaluation may be recommended before elective treatment.

Hyperhidrosis is a well-studied non-cosmetic use of BOTOX.

The strongest evidence applies to primary axillary hyperhidrosis. Evidence also supports palmar treatment, while plantar evidence is more limited.
