Filler Dissolving · Lip Filler Migration · Columbia, Maryland Area

If Your Filler Doesn’t Look Like You Anymore, Start With an Assessment.

Puffiness, a “filler mustache,” blurred lip borders, asymmetry or simply feeling overfilled can be frustrating. The answer is not always more filler—and it is not always dissolving everything.

At Bolus Med Spa, filler correction begins by identifying what may actually be contributing to the appearance, what type of filler was used when known, and whether targeted hyaluronidase, broader correction, observation or a different plan makes sense. When appropriate, we use ultrasound technology to help assess where existing HA filler may be located and support more targeted treatment planning.

Correction Before More Product

The goal is not to erase your face. It is to understand what is there, correct what should be corrected, and rebuild only when it makes sense.

Filler dissatisfaction can have different causes. A clinical evaluation is important before labeling every lump, shadow, fullness or asymmetry as “migration.”

Does This Sound Familiar?

“I think my filler migrated.”

Sometimes that is exactly what happened. Sometimes the appearance has another explanation.

Patients commonly seek correction because the lip border looks less defined, fullness sits above the lip, the face looks puffy, filler feels lumpy or uneven, or a result simply no longer fits the rest of the face. Migration is a recognized potential complication of dermal fillers, but swelling, product placement, tissue changes, nodules and other issues can also affect appearance.

Lip Filler Migration

Fullness Above the Lip

Some patients describe this as a “filler mustache” or a shelf above the lip. Assessment helps determine whether HA filler is contributing and how much correction may be appropriate.

Overfilled Appearance

Too Much Volume

More product is not always better. If proportions have become distorted or heavy, the first step may be correction rather than adding another syringe.

Lumps + Asymmetry

Something Feels Off

Not every irregularity should be treated the same way. Timing, product, location, symptoms and examination matter before deciding what to do next.

Ultrasound-Assisted Filler Assessment

See more before deciding what to dissolve.

Surface appearance does not always tell the entire story. When appropriate, Bolus uses ultrasound technology to help assess the location of existing HA filler and surrounding anatomy.

That additional information can help us plan a more targeted correction rather than assuming every area of fullness needs to be dissolved. Ultrasound does not replace clinical judgment or guarantee that every filler deposit can be identified, but it can provide useful information when planning treatment.

Locate

Better Understand What Is There

Ultrasound can provide additional information about where HA filler may be present and how it relates to the area being evaluated.

Target

Plan a More Focused Correction

When appropriate, ultrasound findings can support more targeted Hylenex treatment rather than treating an area based only on what can be seen or felt from the surface.

Conserve

Avoid Unnecessary Hyaluronidase When Possible

The objective is not to use more dissolver by default. A more selective plan may help avoid unnecessary hyaluronidase exposure when the clinical findings support targeted correction.

What Hyaluronidase Does

HA filler can often be reduced or dissolved. Not every filler can.

Hyaluronidase is an enzyme used in aesthetic medicine to break down hyaluronic-acid filler. At Bolus, we use Hylenex (hyaluronidase) when clinically appropriate for HA filler correction.

Its use for elective HA filler dissolution is off-label. Different HA products and different amounts of filler may respond differently, so there is no universal “one injection dissolves everything” formula. Calcium hydroxylapatite, PLLA and permanent fillers are different materials and are not precisely dissolved with hyaluronidase in the same way.

Targeted Correction

Sometimes Less Is Enough

When appropriate, correction may focus on a specific area rather than automatically attempting to remove every trace of prior HA filler. Ultrasound may be incorporated to support a more selective treatment plan.

Dissolve + Rebuild

Sometimes Resetting Makes More Sense

More extensive correction may be considered when filler is widespread, proportions are distorted or a clean reassessment is needed. The area is allowed to settle and is reassessed before deciding whether rebuilding is appropriate.

Staged Treatment

Reassess Before Rebuilding

Dissolving and immediately replacing filler is not automatically the best strategy. Tissue appearance can change after correction, and a staged plan may provide a clearer endpoint.

An Important Distinction

Correction is different from emergency complication management.

This page is about elective evaluation of filler appearance—not self-diagnosis of an urgent filler complication.

Elective Concern

Migration, unwanted volume, asymmetry, puffiness or dissatisfaction may be evaluated in a scheduled clinical consultation.

Urgent Symptoms

Severe or escalating pain, unusual blanching or dusky skin changes, or visual or neurologic symptoms after filler require urgent medical evaluation. Do not use a website form or routine text pathway for an emergency.

What To Expect

A correction plan should be based on what is actually present.

Assessment

We review the treated area, timing, symptoms, prior products when known, and what specifically bothers you.

Correction Plan

If HA filler dissolution is appropriate, the amount and extent are individualized. More than one session may sometimes be needed.

Reassessment

Once the area settles, we reassess before deciding whether anything should be replaced, rebalanced or simply left alone.

The Bolus Approach

We are not trying to sell you another syringe.

Sometimes the most aesthetic decision is to remove product. Sometimes it is to leave something alone. Sometimes the problem is not filler at all.

Our objective is to restore proportion and help the face look natural, rested and balanced. If rebuilding later makes sense, we look at the whole face rather than automatically recreating the same treatment that brought you in for correction.

Natural Results

Correct First. Refill Carefully.

A correction visit is not a promise that more filler will follow. Any future treatment should earn its place in the plan.

Full-Face Perspective

What You Asked To Fix May Not Be the Whole Story

Lips, chin, temples, midface and lower-face proportions influence one another. After correction, facial balancing may reveal a more strategic path forward.

Filler Correction Investment

Transparent pricing. Individualized treatment.

The amount of correction needed can vary substantially depending on the area, product, volume, age of the filler and whether treatment needs to be staged.

Typical Filler Dissolving Investment
$350 + $3 per unit of Hylenex

$350 treatment fee plus $3 for each unit of Hylenex used.

Total investment depends on the amount of Hylenex clinically appropriate for the correction and whether treatment requires more than one session. We will review the recommended treatment and anticipated investment before proceeding.

After Correction

Rebuilding should be a plan—not a reaction.

If you eventually want to restore volume or continue with injectables, we can build a facial-balancing roadmap based on your anatomy and priorities rather than automatically putting filler back where it was removed.

Private Client

Planning multiple treatments over the year?

For patients whose correction becomes part of a broader injectable, biostimulator, wrinkle-relaxer or skin-treatment roadmap, Private Client may provide preferred pricing, Bolus Wallet contributions and a more coordinated annual treatment strategy.

It is not necessary for filler correction, and it should only be considered when it makes sense for your larger plan.

Common Questions

Filler dissolving is not as simple as social media makes it sound.

Why does Bolus use ultrasound for filler correction?

When appropriate, ultrasound can provide additional information about where HA filler may be located and how it relates to the area being evaluated. This can support more targeted treatment planning and may help avoid unnecessary hyaluronidase exposure when selective correction is appropriate. Ultrasound does not guarantee that every filler deposit can be identified.

How much does filler dissolving cost at Bolus?

Filler dissolving is $350 plus $3 per unit of Hylenex used. The total varies according to the amount of Hylenex clinically appropriate and whether correction requires more than one treatment session.

Can all filler be dissolved with hyaluronidase?

No. Hyaluronidase is used to break down hyaluronic-acid filler. Other filler materials such as calcium hydroxylapatite, PLLA and permanent materials are different and cannot simply be treated as if they were HA filler.

Does everything have to be dissolved?

Not necessarily. Elective correction can be targeted or broader depending on the clinical findings and the goal. There is no universal approach.

Will I look exactly like I did before filler?

That cannot be guaranteed. The face may have changed since the original treatment, and swelling, tissue behavior, prior filler volume and other factors can affect how the area looks after correction.

Can filler be replaced after dissolving?

Sometimes, but replacement should be based on reassessment rather than an automatic refill. A staged approach may be recommended.

Is hyaluronidase FDA-approved specifically for cosmetic filler dissolving?

No. Hyaluronidase is used off-label in aesthetic medicine to dissolve HA filler. Risks, alternatives and expectations should be reviewed before treatment.

Can dissolving filler have side effects?

Yes. Treatment can cause temporary swelling, bruising, discomfort and other reactions. Allergic reactions are possible. Your clinician should review risks and relevant history before elective treatment.
