Stop chasing breakouts.
Start understanding your skin.

If you feel like you've tried everything, the problem may not be that you need another product. You may need a better strategy.

At Bolus Med Spa in Elkridge, near Columbia, we approach acne as a condition to understand and manage—not a pimple to chase one appointment at a time.

Active acne • Adult acne • Hormonal patterns • Congestion • Post-acne marks • Acne scars
Acne Is Not One Problem

Your skin may be breaking out for more than one reason.

Oil production, follicular congestion, inflammation, microbial factors, hormones, products, medications, lifestyle, and other health patterns can overlap. That is why copying someone else's routine—or repeatedly treating the surface without looking at the pattern—can be frustrating.

We don't start by asking, “Which machine should we use?” We start by asking, “What is your skin telling us?”
What We Look At

Because “acne” can look very different from one person to the next.

Your treatment plan depends on what is active now, what is driving recurrence, what your skin has been exposed to, and what acne has already left behind.

01

Inflammation

Red, tender papules, pustules, deeper lesions, and persistent inflammatory activity require a different strategy than simple congestion.

02

Oil + Congestion

Blackheads, whiteheads, clogged pores, product buildup, and excess sebum can create a cycle that needs both home care and professional management.

03

Hormonal Patterns

Jawline flares, cyclical acne, adult-onset acne, irregular cycles, androgen-related symptoms, and PMOS may be clues that the skin is reflecting a broader endocrine pattern.

04

Microbial + Follicular Factors

Not every acne-like eruption behaves the same. Bacterial acne and yeast-associated follicular eruptions can require different thinking.

05

Your Skin Routine

Over-exfoliation, occlusive products, inconsistent retinoid use, barrier disruption, and the wrong combination of actives can keep skin irritated or congested.

06

What Acne Left Behind

Post-inflammatory redness, dark marks, uneven pigment, enlarged pores, and true acne scars are separate problems—and we treat them differently.

Comprehensive Skin Analysis

We build the plan before we build the package.

The goal is to understand the pattern of your acne, your skin type, what you've already tried, your tolerance for downtime, and the marks or scars that may need attention after active breakouts are controlled.

For selected patients, advanced skin imaging can help us document visible and underlying concerns and create a more objective baseline for long-term skin management.

✓ Active breakout pattern + severity
✓ Skin type, sensitivity + pigment risk
✓ Current products, prescriptions + prior treatments
✓ Hormonal and cyclical clues when relevant
✓ Post-inflammatory redness + hyperpigmentation
✓ Scar type: rolling, boxcar, ice-pick or mixed
✓ Home-care strategy + treatment sequencing
The Bolus Acne Strategy

Clear it. Correct what it left behind. Keep it controlled.

Acne treatment works better when each stage has a job. We may use one modality or strategically combine several based on your skin.

Phase One

Control Active Acne

Reduce active inflammation, congestion, oil-related factors, and recurrent breakouts with the appropriate combination of professional treatments and home care.

Phase Two

Correct the Evidence

Once the skin is more stable, address persistent redness, post-inflammatory hyperpigmentation, uneven tone, texture, and early scarring.

Phase Three

Protect the Result

Acne can recur. Long-term success often requires a maintenance strategy rather than stopping everything the moment the skin clears.

Our Treatment Toolbox

We are not married to one machine.

The advantage of a multi-modality practice is that the treatment can be selected for the skin—not the other way around.

Aerolase Neo

A 1064 nm Nd:YAG platform that can be useful in acne strategies, particularly when we want a laser option that can be used across a broad range of skin tones. Explore Aerolase →

Forever Clear BBL

Sciton's multi-pass BroadBand Light acne protocol uses blue, yellow, and infrared light to address acne-associated bacteria, inflammation/redness, and healing. It can be an excellent option for appropriately selected patients. Explore BBL →

Peels + Extractions

For selected congested or acne-prone skin, professional exfoliation, chemical peels, HydraFacial, and targeted extractions may be incorporated to support pore management and skin turnover.

Professional Skincare

Your home routine is part of the treatment—not an afterthought. We may build cleanser, exfoliation, retinoid, pigment control, moisturizer, and sunscreen into the plan based on what your skin actually needs.

Pigment + Red Marks

Once active acne is controlled, persistent discoloration may need its own strategy. Depending on skin type and diagnosis, options may include professional skincare, peels, MOXI, BBL, or other carefully selected modalities.

Acne Scars

True scars are not simply “dark spots.” Scar treatment may require combinations such as subcision, TCA CROSS, microneedling, RF microneedling, PRP/PRF, HALO, or deeper resurfacing depending on scar architecture. Explore Acne Scar Treatment →

Results Ecosystem

Sometimes the best acne “package” isn't five identical treatments.

For patients who need a more comprehensive reset, we can build a results-focused plan that combines the appropriate technology with skin analysis, professional skincare, supportive treatments, and a maintenance strategy.

Active-Acne Treatment
Professional Skincare
HydraFacial / Extractions When Appropriate

The point is not to sell you more things. The point is to stop leaving out the pieces that determine whether your skin actually improves.

When Acne May Be Telling Us Something More

Skin can be part of a bigger picture.

Some patterns of persistent adult acne—especially when accompanied by irregular cycles, unwanted facial hair, scalp hair changes, weight or metabolic concerns, or other androgen-related symptoms—may warrant evaluation beyond the skin itself.

Polyendocrine Metabolic Ovarian Syndrome (PMOS), formerly known as PCOS, is one example of a broader endocrine-metabolic condition that can affect the skin. When the history suggests a medical driver, we may recommend appropriate medical evaluation rather than pretending another facial or laser will solve the entire problem.

We also consider the broader history when acne-like eruptions behave unusually, including medication, product, lifestyle, gastrointestinal, and yeast/fungal-associated patterns when clinically relevant.

Bolus Med Spa's acne page is educational and does not replace medical evaluation or diagnosis. Hormonal or systemic concerns may require evaluation through an appropriate medical practice.

You deserve more than a treatment menu.

“The question isn't which laser I own. It's which combination makes sense for the skin sitting in front of me.”

Bolus is built around long-term relationships. We want to know how your skin responds, what triggers recurrence, what worked, what didn't, and what the next stage should be. That continuity matters when you're managing a condition that can change over time.

Our goal is not to keep you dependent on endless appointments. It is to give you a thoughtful plan, use technology when it adds value, and help you maintain skin that feels controlled, healthy, and elegant.

Acne FAQs

The questions people ask when they're tired of guessing.

I've tried everything. Why am I still breaking out?

Because acne is not always driven by one factor. Inflammation, oil, congestion, hormones, products, microbial factors, medications, and skin-barrier disruption can overlap. Our first job is to understand the pattern before deciding what belongs in your plan.

Do I need a laser for acne?

Not necessarily. Laser or light treatment can be valuable for selected patients, but it is one tool. Your plan may also involve professional skincare, peels, extractions, HydraFacial, medication evaluation through an appropriate medical provider, or a combination strategy.

Which is better for acne: Aerolase Neo or Forever Clear BBL?

There is no universal winner. Skin type, acne pattern, inflammation, pigment risk, prior treatment response, and treatment goals matter. Having access to both platforms allows us to select rather than force a single technology onto every patient.

Can you treat acne in darker skin tones?

Yes, but treatment selection matters. Darker skin can be more prone to post-inflammatory hyperpigmentation after inflammation or inappropriate energy-based treatment. We choose modalities, settings, and sequencing with skin type and pigment risk in mind.

What about hormonal acne?

Hormonal patterns can show up as recurrent adult acne, often involving the lower face or jawline and sometimes changing with the menstrual cycle. If your history suggests a broader endocrine issue, we may recommend medical evaluation in addition to treating the visible skin.

Is PCOS now called PMOS?

Yes. In 2026, an international consensus renamed Polycystic Ovary Syndrome (PCOS) as Polyendocrine Metabolic Ovarian Syndrome (PMOS) to better reflect its broader endocrine and metabolic effects. Because the older term remains familiar, you may see both terms during the transition.

What about “fungal acne”?

Some acne-like eruptions can involve yeast-associated folliculitis and may not respond like conventional acne. “Fungal acne” is a popular term, but an accurate diagnosis matters because treatment can differ. Persistent, itchy, uniform bumps or an unusual treatment response are reasons to look more carefully rather than simply escalating acne products.

Can gut health cause acne?

The relationship between diet, the gut microbiome, inflammation, and acne is an evolving area of research. We do not diagnose every breakout as a “gut problem.” We do, however, consider the broader health history when it is relevant and avoid pretending skin exists in isolation from the rest of the body.

Can you treat the dark marks acne leaves behind?

Yes. Post-inflammatory hyperpigmentation and post-inflammatory redness are different from active acne and from true scars. Once breakouts are controlled, we can build a targeted strategy for residual discoloration using skincare, peels, and selected laser or light treatments when appropriate.

Can you treat acne scars too?

Yes. Scar treatment depends on scar type and depth. Rolling, boxcar, and ice-pick scars may require different approaches. Options can include subcision, TCA CROSS, microneedling, RF microneedling, PRP/PRF, HALO, or deeper resurfacing. Often the strongest plan is a combination rather than one device. Explore our Acne Scar Treatment approach →

How many treatments will I need?

There is no responsible one-number answer without seeing your skin. Active acne often benefits from a series followed by maintenance, while pigment and scars may require separate phases. We will explain the expected sequence before you commit to a plan.

Do I need to know which treatment to book?

No. Tell us what is happening with your skin. Choosing between Aerolase, BBL, peels, skincare, scar treatments, or combinations is our job.
