Rescheduling & Cancellation Policy
This Cancellation and No-Show Policy outlines Bolus Med Spa’s (“we,” “our,” or “the Company”) practices with respect appointments booked at our practice.
Our patients are the most important part of our practice. This cancellation/no show policy is put in place for our patients to ensure that you get the times you want. Our appointments are reserved just for you. Related to this, we want you to understand that it is also a potential appointment for someone else who may have wanted that particular time.
As a courtesy to our providers and to other patients, we ask that you provide our office a minimum of 24 hours’ notice should you need to cancel or reschedule your appointment. by simply logging into your portal and canceling or rescheduling your appointment. If an appointment is NOT canceled or rescheduled within 24 hours of appt time, or a no-show, you will be assessed a $75 "No Show" fee. You may text us at 443.776.8531 and we will try to fill your spot.
If you arrive to your appointment more than 10 minutes past the originally scheduled time, we will consider you a "No Show" and may require you to reschedule to avoid impacting other clients’ appointments. You will be charged the cancellation fee of $75.
Additionally, VIP Members who are a NO Show will forfeit their promotional treatment.The monthly payment will be available for other treatments or retail products.
Groupon customers who miss their appointments by default of a no call/no show will forfeit the promotional pricing of their Groupon which means you will pay full price. Additionally you will be charged the appropriate cancellation or no show fee of $75.
THERE ARE NO EXCEPTIONS! We do no play favorites. Please do not ask to refund the fee. Life Happens.
Self SCHEDULING APPOINTMENTS
Appointments are required for any and all appointments. We are unable to accept walk-ins.
For your convenience we are extremely proud to offer self scheduling. This also means that patients are required to cancel or reschedule any appointment in their portal. The portal is available 24/7 for optimal convenience and efficiency. This HIPAA compliant portal is located at www.bolusmedspa.com and labeled "Patient Portal". Failing to reschedule your appointment will result in a "No-Show" fee of $75. If you haven't setup the portal, the first thing to do is check your email for the appointment confirmation you made, then setup a password and confirm the account. You can then you can login and make appointment changes.
All patients receive an email invitation to setup a password to their portal immediately when any appointment is made. This allows all patients the convenience of self scheduling and managing their appointments.
Expiration
All treatments and service purchases expire 12 months after the date of purchase. Your payment will not be transferable or used for credit towards other services, products, or treatments.
Retail products may be returned un-opened in original packaging within 30 days. NO Exceptions!
Gift Cards expire within one year of purchase.
VIP Membership
Active membership required to redeem treatments.
While this is a year round spa membership, we do require a 12 month commitment for all memberships. If you cancel prior to the 12 month term, you will be required to pay an early cancellation fee related to your membership level.
Tox: Repay All Discounts
Silver: $750 fee
Gold: $1000 fee
Platinum: $2000 fee
No Call/No Show (per office policy) will result in $75 fee and LOSS OF YOUR PROMOTIONAL MONTHLY TREATMENT.
All Cancellations should be sent to getbolused@gmail.com and text 443.776.8531. Receipt acknowledge required to ensure you do not lose your treatments or voucher. If you cancel without our knowledge we cannot help you.
Active Membership Required to Redeem Treatments or Membership dues.
All promotional treatments or membership dues must be used in full at time of cancellation or they will expire.
Cannot combine membership with other discounts or offers.
Promotional value or treatments expires after 90 days, but you will retain dues paid towards other treatments or services as long as membership is active.
Once membership is cancelled you will owe Bolus Med Spa nothing and Bolus Med Spa will owe you nothing.
Pausing membership is not an option.
If you cancel we will determine if you are a good fit to re-join.
Any previous treatments or dues not redeemed while active will be voided.

Bolus Med Spa’s Refund Policy
Introduction
This Refund Policy outlines Bolus Med Spa’s (“we,” “our,” or “the Company”) practices with respect to information collected from users who access our website at bolusmedspa.com (“Site“), or otherwise share personal information with us (collectively: “Users”).
TREATMENTS OR PACKAGES
Treatments: No Refunds. Treatments or services are non-transferable and must be used within one year from the date of purchase unless otherwise part of your treatment plan.
Packages: No refunds. Packages are non-transferable and must be used within one year from the date of purchase unless otherwise part of your treatment plan.
Order Cancellations

Prescription orders that are submitted online are processed immediately and may not be canceled.

Orders for retail will be evaluated on an individual basis. Please be aware that The BolusMed Spa has the right to deny order cancellation.
Returns
Once a retail item is delivered to you, you can return that item within 30 days of receipt as long as it is unused, unopened, and sealed in the same condition that you received it, and must be in the original packaging. Prescription medications are not eligible for a return, refund or exchange.
Bolus Med Spa’s Consultation Policy
New patients may receive ONE free virtual consultation. Future consults will be $150.
Additional consults will be $150.
The consult fee will go towards your treatment if booked and treated within 5 days. If you reschedule the treatment you lose the $150 credit.
In person consultations are $150. If booked and treated within 5 days the fee goes towards the treatment.

This Privacy Policy outlines Bolus Med Spa’s (“we,” “our,” or “the Company”) practices with respect to information collected from users who access our website at bolusmedspa.com (“Site“), or otherwise share personal information with us (collectively: “Users”).
Grounds for data collection
Processing of your personal information (meaning, any information which may potentially allow your identification with reasonable means; hereinafter "Personal Information") is necessary for the performance of our contractual obligations towards you and for providing you with our services, to protect our legitimate interests and for compliance with legal and financial regulatory obligations to which we are subject.

What types of information do we collect?
We collect two types of data and information from Users.

We are not aware of the identity of a User from which the Non-personal Information was collected. We receive your Personal Information from various sources:
- When you voluntarily provide us with your personal details in order to register on our Site;
- When you use or access our Site in connection with your use of our services;
- From third-party providers, services, and public registers (for example, traffic analytics vendors).
What information do we collect?
We do not rent, sell, or share Users’ information with third parties except as described in this Privacy Policy.

Retention
We will retain your personal information for as long as necessary to provide our services, and as necessary to comply with our legal obligations, resolve disputes, and enforce our policies. We take great care in implementing and maintaining the security of the Site and your information. Although we take reasonable steps to safeguard information, we cannot be responsible for the acts of those who gain unauthorized access or abuse our Site, and we make no warranty, express, implied, or otherwise, that we will prevent such access.
Transfer of data outside the EEA

Out of respect to your right to privacy, we provide you with such marketing materials as means to decline receiving further marketing offers from us. If you unsubscribe, we will remove your email address or telephone number from our marketing distribution lists.
Please note that even if you have unsubscribed from receiving marketing emails from us, we may send you other types of important email communications without offering you the opportunity to opt out of receiving them. These may include customer service announcements or administrative notices.
Corporate transaction
We may share information in the event of a corporate transaction (e.g. sale of a substantial part of our business, merger, consolidation, or asset sale). In the event of the above, the transferee or acquiring company will assume the rights and obligations as described in this Privacy Policy.
Minors
We understand the importance of protecting children’s privacy, especially in an online environment. The Site is not designed for or directed at children. Under no circumstances shall we allow the use of our services by minors without prior consent or authorization by a parent or legal guardian. We reserve the right to periodically amend or revise the Privacy Policy; material changes will be effective immediately upon the display of the revised Privacy policy. Your continued use of the Platform, following the notification of such amendments on our website, constitutes your acknowledgment and consent of such amendments to the Privacy Policy and your agreement to be bound by the terms of such amendments.
