Under-Eye Botox + Jelly Roll Treatment - Columbia, Maryland

Under-Eye Botox + Jelly Roll Treatment
Smoother When You Smile. Still Natural.

Under-eye Botox and other wrinkle relaxers can soften the fine lines that fan outward from the corners of the eyes by reducing selected orbicularis oculi muscle activity while preserving a natural, expressive smile.

Crow’s feet lines can make the upper face look more tired or aged than you feel. The goal at Bolus is a smoother, more refreshed eye-area without creating a heavy, flat or overdone appearance.

At Bolus Med Spa, crow’s feet treatment begins with your smile pattern, eye shape, cheek movement and anatomy. The goal is not to erase every expression line. It is to soften the lines that make the eye area look more tired or aged while preserving a natural, refreshed appearance that still looks like you. Bolus Med Spa is located in Elkridge, Maryland, just minutes from Columbia and Ellicott City, and serves patients throughout Howard County and surrounding communities.

Why Crow’s Feet Lines Form

Every eyebrow raise folds the eye-area skin.

Horizontal crow’s feet lines commonly develop from repeated contraction of the orbicularis oculi muscle, the broad muscle that elevates the eyebrows.

They are often called crow’s feet wrinkles, crow’s feet lines or horizontal crow’s feet lines.

When you raise your brows, the frontalis contracts and the overlying skin folds. Over time, lines that initially appear only with expression may become visible even when the eye-area is relaxed.

That distinction matters because treating movement is different from correcting an established crease in the skin.

Crow’s Feet Lines + Horizontal Crow’s Feet Wrinkles

A common concern. A more complex treatment area than it looks.

Crow’s feet are the fine lines that radiate from the outer corners of the eyes. They are strongly influenced by repeated contraction of the orbicularis oculi muscle when you smile or squint. Sun exposure, skin quality and loss of collagen can make them more visible over time.

01 - Dynamic

Lines When You Raise Your Brows

Early crow’s feet lines may appear primarily when you raise your eyebrows. Carefully reducing selected frontalis activity can soften repetitive folding while maintaining useful facial expression.

02 - Resting

Lines Visible At Rest

With repeated movement and changes in skin quality, horizontal creases may remain visible even when your eye-area is relaxed. A neuromodulator can reduce the muscular force, but an established skin crease may not disappear completely from injections alone.

03 - Balance

Brow Position Matters

The frontalis is an important brow elevator. Brow height, eye-area length, eyelid anatomy, asymmetry and how strongly you recruit the eye-area all influence treatment. That is why eye-area injections should be mapped to the individual rather than copied from a standard pattern.

Under-Eye Botox + Jelly Roll

Not every under-eye line is the same.

“Jelly roll” describes a small band or bulge directly beneath the lower eyelashes that becomes more noticeable when some people smile or squint. In selected patients, it is driven largely by contraction of the lower portion of the orbicularis oculi muscle.

Very conservative neuromodulator treatment may soften that muscle-related bunching. But under-eye Botox is not a universal treatment for under-eye wrinkles, bags, hollows or loose skin. Identifying what is actually creating the concern is the most important part of the evaluation.

Jelly Roll

A dynamic muscle bulge immediately beneath the lower lashes that becomes more visible with smiling or squinting may respond in carefully selected patients.

Under-Eye Wrinkles

Fine lines can come from movement, skin quality, sun damage, collagen loss or cheek compression. Botox will not address every cause.

Bags + Hollows

Orbital fat, fluid, volume loss and tear-trough anatomy are different problems. A jelly-roll injection does not remove fat bags or restore lost volume.

Your Treatment At Bolus

A small area can require thoughtful planning.

The eye-area and glabella work together to influence brow position and upper-face expression. We evaluate the entire upper face rather than treating horizontal lines in isolation.

01 - Evaluate

Watch You Move

We evaluate your resting brow position, eyebrow asymmetry, eye-area height, frontalis recruitment, muscle strength and any pre-existing eyelid or brow heaviness.

02 - Plan

Choose The Pattern

Injection sites, dose and product are selected according to your anatomy and treatment objective. Some patients want subtle softening and maximal movement. Others prefer a smoother eye-area. Natural and refreshed should never mean one identical result for everyone.

03 - Reassess

Let The Result Develop

The effect develops gradually rather than instantly. We want enough time for the neuromodulator response to declare itself before deciding whether additional treatment is appropriate.

Advanced Eye-Area Assessment

Jelly roll Botox is a narrow-indication treatment.

Cosmetic botulinum toxin injection directly beneath the lower eyelid is an off-label technique. The lower eyelid contributes to blinking, eye closure and support of the ocular surface, so excessive weakening can create unwanted functional or aesthetic changes.

Potential risks can include bruising, asymmetry, altered smile dynamics, dry-eye symptoms, lower-lid weakness or retraction, increased scleral show and, rarely, ectropion. Patients with baseline lower-lid laxity, significant dry eye, prominent bags or anatomy that depends on orbicularis tone may not be appropriate candidates.

Movement Test

We evaluate the area at rest and with a full smile or squint to determine whether the concern is truly created by lower-eyelid muscle contraction.

Conservative Treatment

The lower eyelid is not an area where “more” is better. When treatment is appropriate, conservative planning helps protect natural expression and lower-lid function.

Alternative Plan

If the concern is primarily skin texture, laxity, volume loss or fat prominence, another treatment—or no injectable treatment—may make more sense.

When Botox Is Not Enough

Loose, crepey under-eye skin needs a different conversation.

Under-eye Botox works on muscle movement. It does not tighten loose skin, rebuild lost collagen or resurface etched wrinkles. When the lower-eyelid skin has become significantly crepey, lax or heavily wrinkled at rest, reducing muscle movement alone may provide limited improvement and may not be the treatment we recommend.

This is an important distinction in our approach at Bolus. A small jelly roll caused primarily by muscle contraction may be an injectable problem. Wrinkles created by skin laxity, collagen loss and photoaging are often better approached as a skin-quality problem.

Botox: Movement

Best suited to carefully selected dynamic concerns driven substantially by muscle contraction. The effect is temporary and maintenance is required if you want to preserve the result.

CO2: Resurfacing + Tightening

Fractional ablative CO2 resurfacing can address static wrinkles, texture and selected degrees of lower-eyelid skin laxity by creating controlled remodeling of the skin. It is a more intensive treatment with greater healing and downtime than Botox.

Erbium: Precision Resurfacing

Er:YAG resurfacing can be used to improve fine lines, texture and photoaged skin with highly water-absorptive laser energy. Treatment depth and intensity are individualized to the condition of the skin and the desired recovery profile.

Short-Term Movement Control vs Longer-Term Skin Remodeling

Sometimes the better investment is rebuilding the skin.

Neuromodulators and laser resurfacing solve different problems. Botox temporarily reduces selected muscle activity. Ablative resurfacing is intended to create a longer-term remodeling response in the treated skin, although aging continues and no laser result is permanent.

For patients with more advanced resting wrinkles or lax lower-eyelid skin, we frequently discuss CO2 or Erbium laser resurfacing instead of—or in selected cases as part of a plan with—neuromodulator treatment. The appropriate option depends on skin type, degree of laxity, wrinkle depth, eye and eyelid anatomy, medical history, healing factors, downtime tolerance and treatment goals.

Significant eyelid laxity, fat prolapse, ectropion risk, structural bags or excess skin may require evaluation beyond cosmetic laser or injectable treatment. Our goal is to recommend the modality that matches the anatomy rather than forcing every under-eye concern into an injection.

Eye-Area Movement vs Etched Lines

A smooth eye-area is not only about toxin.

If your crow’s feet lines are visible even when you are completely relaxed, the skin may have developed resting creases from years of repeated folding, sun exposure, collagen loss and other skin-aging factors.

Dynamic Frown Lines

Horizontal lines that appear mainly when you raise your eyebrows often respond well when frontalis activity is carefully reduced.

Etched Frown Lines

A eye-area crease that is already etched into the skin may soften after repeated reduction in movement, but resurfacing, collagen-focused treatments or skincare may also be considered depending on depth and skin quality.

What To Expect

What to expect from under-eye Botox.

Treatment Time

The injection portion is generally brief. Consultation and facial assessment are part of the appointment.

Downtime

Most patients return to normal daily activities immediately. Temporary redness, tenderness, small bumps or bruising can occur.

Onset

Changes often begin within several days. The full effect is not immediate and varies by product and individual response.

Duration

Neuromodulator effects are temporary. Many patients experience results lasting several months, although duration varies with product, dose, muscle strength and individual biology.

Candidacy + Eye-Area Safety

Not everyone should be treated the same way.

A cosmetic injection is still a medical treatment. Your health history, medications, anatomy and prior response matter.

Tell us before treatment if you...

  • Are pregnant, planning pregnancy or breastfeeding.
  • Have a known neuromuscular disorder or significant muscle weakness.
  • Have had a prior reaction to a botulinum toxin product.
  • Have an infection, rash or significant inflammation near the proposed injection sites.
  • Take medications that may affect neuromuscular transmission, bleeding or bruising.
  • Have prior eyelid or brow drooping, facial weakness, facial surgery or significant asymmetry.

Potential risks

  • Pain, tenderness, redness, swelling or bruising at injection sites.
  • Headache or temporary discomfort.
  • Asymmetry or an undesired change in expression.
  • Temporary eyebrow or eyelid drooping.
  • Temporary weakness in nearby muscles.
  • Rarely, toxin effects can spread beyond the injection area and cause serious symptoms such as generalized weakness or difficulty speaking, swallowing or breathing.

Clinical evaluation matters.

Wrinkle relaxers should not be selected solely by the number of units someone received elsewhere or by a social-media injection map. Different products have different dosing systems, and the safest treatment pattern depends on the individual face.

Under-Eye Botox FAQ

Common questions about jelly roll Botox.

What is jelly roll Botox?

Jelly roll Botox is an informal name for small, carefully placed neuromodulator injections used to reduce a muscle-related bulge immediately beneath the lower eyelid that appears with smiling or squinting. It is an off-label cosmetic technique.

Is under-eye Botox the same as crow’s feet Botox?

No. Crow’s feet treatment targets the lateral orbicularis oculi beside the eyes. Jelly roll treatment is placed beneath the lower eyelid and has different anatomical considerations and risks.

Can Botox fix under-eye bags?

Not generally. True under-eye bags may reflect orbital fat, fluid, skin laxity or other anatomy. Weakening the lower-eyelid muscle may be inappropriate in some patients and can potentially make certain concerns more noticeable.

Can under-eye Botox treat fine lines?

It may soften selected dynamic lines caused substantially by muscle contraction, but many under-eye lines are related to skin quality, sun damage, volume changes or cheek movement. Those concerns may require a different approach.

What are the risks of jelly roll Botox?

Risks can include bruising, asymmetry, altered smile dynamics, dry-eye symptoms, lower-lid weakness or retraction, increased scleral show and other unwanted changes. Patient selection and conservative treatment are especially important in this area.

Is Botox or laser better for under-eye wrinkles?

It depends on what is causing the wrinkles. Botox primarily reduces selected muscle movement and may be appropriate for a dynamic jelly roll in carefully selected patients. If wrinkles are etched at rest or the lower-eyelid skin is crepey or lax, resurfacing with an ablative CO2 or Erbium laser may address the skin itself more directly. Some patients may benefit from a staged combination plan. Significant structural eyelid laxity or excess skin may require a different evaluation.

How much does under-eye Botox cost?

At Bolus, typical under-eye / jelly roll Botox treatment is approximately $100–$150. Exact cost depends on the product, anatomy, treatment plan and individualized dose. Botox, Jeuveau and Xeomin Standard pricing is $15 per unit, with Foundation Private Client pricing at $10 per unit and Elevated Private Client pricing at $9 per unit. Dysport uses its own product-specific dosing.
